All fields are required with the exception of the DocuStor box number. However, we strongly recommend you enter this number, if known, as it will help us process your request much quicker.

In the file number and file name fields please reference any information that may help us locate your record such as (Name on file, SSNs, id numbers,folder name, etc.)

If one of the fields are not used please type "NA" or "Unkown" in the field NOT USED.
